Office Visit Guidelines

1. Complete the Patient information/ Demographic forms. We have linked the forms below to print and bring with you to save time.

  • Cultural Competency form
  • Medical History Form 1
  • Medical History Form 2

2. If you have a prior primary care physician, please fill out the form requesting that he/he send your medical records to us.

3. Please bring all medications with you that you are currently taking.

4. Please bring your current insurance information and I.D. to your first appointment. This information is required for verification.

-Please bring the following:

• Insurance card (s)

• Driver’s License or any legal form of government I.D.

5. If you have a co-payment with your insurance, it must be paid at the time of your office visit. We only accept cash, we do not accept credit cards or checks.

6. Before you leave, make sure you schedule a follow-up visit, if needed.

7. You are responsible for picking up all referrals from our office.

8. After any blood tests or diagnostic tests, you must call the office within three business days for the results.
